Does Stevia Break a Fast?
Quick answer: The steviol glycosides themselves carry no meaningful calories, so under a fat-loss fast stevia is one of the safer choices. Two catches: the packet is mostly not stevia, and the human insulin evidence is genuinely mixed — one careful trial in the fasted state found stevia in water raised C-peptide, while a 26-study meta-analysis found no significant effect on insulin at all. For a strict fast, skip it.
Stevia gets treated as the natural, guilt-free option in fasting circles. That reputation is roughly deserved and considerably less certain than it is stated. Here is what is actually established.
What stevia is, and what the FDA actually says
Stevia sweetness comes from steviol glycosides — Rebaudioside A (Reb A) and stevioside are the ones you will see named. The FDA reports them as 200 to 400 times sweeter than table sugar.
The regulatory status is worth getting right, because almost every article gets it wrong. High-purity steviol glycosides are not approved food additives. They went through the FDA's GRAS notification program, and in the agency's own words it "has not questioned the notifiers' GRAS conclusions for these high-purity stevia derived sweeteners under the intended conditions of use." That is a meaningfully weaker statement than approval, and it applies only to preparations of at least 95% purity.
The flip side matters too: whole stevia leaf and crude stevia extracts are not GRAS, and the FDA does not permit their import for use as sweeteners. The "just use the natural leaf" advice you see online describes something that is not legal to sell as a sweetener in the US.
The acceptable daily intake for Reb A is 12 mg per kg of body weight per day, derived from the Joint FAO/WHO expert committee's ADI of 4 mg/kg for steviol glycosides expressed as steviol equivalents. The FDA estimates a 60 kg (132 lb) person would need 27 tabletop packets a day to reach it.
The packet is mostly not stevia
This is the part that actually decides the fasting question, and it is arithmetic rather than opinion.
The FDA assumes a sweetener packet is about as sweet as two teaspoons of sugar — roughly 8 g. At 200 to 400 times the sweetness of sugar, the steviol glycoside needed to do that is 20 to 40 milligrams. That is a quantity you could not see in your palm.
So what is the rest of the packet? Bulking agent — commonly a sugar alcohol like erythritol, or a carbohydrate like dextrose or maltodextrin. It exists so that the packet pours like sugar.
And this is where the label stops helping you. Under 21 CFR 101.9, calories "less than 5 calories may be expressed as zero" per serving. A packet carrying a gram of carbohydrate bulker is legitimately labeled zero calories. Three packets across a morning are still labeled zero and are no longer nothing.
The practical rule: liquid stevia drops are the closest thing to pure steviol glycoside and carry the least hidden material. Packets and "stevia blends" are mostly filler, and the filler is what you are actually consuming.
Three fasts, three answers

A strict clean fast — treat it as broken. A strict fast is zero calories and zero inputs. A bulked packet is not zero, and the whole point of a strict fast is not having to argue about it.
A fat-loss fast — stevia is one of the better options. Pure steviol glycosides add no meaningful energy, and the meta-analytic evidence below finds no significant insulin effect. If a few drops in black coffee is what keeps you inside your window until lunch, that is a good trade.
An autophagy fast — unknown, so no. There is no human evidence on sweeteners and autophagy in either direction, and no test that could tell you. If autophagy is the specific goal, the sensible position is water and black coffee.
What the human evidence actually shows
The honest summary is that stevia has been studied more than most sweeteners and the results do not line up neatly.
The most-cited study is weaker than its reputation. Anton and colleagues, in Appetite in 2010, gave 31 adults — 19 lean, 12 with obesity — preloads before lunch and dinner across three test days. Stevia preloads reduced postprandial glucose compared with sucrose (p<0.01) and reduced postprandial insulin compared with both aspartame and sucrose (p<0.05). Participants did not compensate by eating more later.
That is a real finding, and it is regularly misused. The stevia and aspartame preloads were 290 calories each — this was a study about substituting stevia for sugar in food, not about taking stevia in a fasted state. It tells you nothing about a few drops in black coffee at hour 14.
The study that does test the fasted state cuts the other way. Sambra and colleagues, in Clinical Nutrition ESPEN in 2022, gave 33 women with insulin resistance 60 mL of water containing just 15.3 mg of stevia in a randomized crossover. The stevia condition produced a higher C-peptide response — an incremental AUC of 1033, against 794 for d-tagatose (P=0.001) and 730 for the control (P=0.012). Serum glucose was also higher at 30 and 60 minutes.
C-peptide is co-secreted with insulin, so a higher C-peptide response means more insulin was released. This is the closest published test to "stevia in water while fasting," and it did not find nothing.
The pooled evidence finds no insulin effect. A 2024 meta-analysis covering 26 studies and 1,439 participants found stevia lowered blood glucose slightly (weighted mean difference −3.84; 95% CI −7.15 to −0.53; P=0.02) on low-certainty evidence, and found no significant effect on insulin concentration (very low certainty) or on HbA1c.
And the acute pooled evidence says these drinks behave like water. A 2023 network meta-analysis of 36 trials in 472 participants — covering stevia, sucralose, aspartame, acesulfame K, saccharin, cyclamate and blends — concluded that non-nutritive sweetened beverages "have no acute metabolic and endocrine effects, similar to water," at low to moderate certainty. Worth knowing when you weigh it: that research group carries extensive food-industry funding disclosures.
How to hold all four at once: the small, single-dose, fasted-state signal in one trial of 33 insulin-resistant women is not contradicted so much as outweighed by pooled data across 26 and 36 trials. The reasonable conclusion is that any insulin effect of stevia is small, inconsistent, and probably depends on who you are — not that it is zero, and not that it is a spike.

What the WHO guideline does and does not say
In May 2023 the WHO issued a guideline recommending "that non-sugar sweeteners not be used as a means of achieving weight control or reducing the risk of noncommunicable diseases." It names stevia and stevia derivatives explicitly. It is a conditional recommendation resting on low-certainty evidence, and it does not apply to people with existing diabetes.
That guideline is frequently waved around in fasting discussions as proof that sweeteners "break a fast." It says nothing of the kind. It is about long-term weight control and disease risk, not about insulin during a fasting window, and it is not a safety finding. Cite it for what it is.
The practical version
- Fasting strictly or for autophagy: water, black coffee, plain tea. No sweetener.
- Fasting for fat loss: a few drops of liquid stevia is a reasonable choice. Prefer drops over packets — you are avoiding the bulking agent, not the stevia.
- Watch the count, not the label. "Zero calories" is a rounding rule, not a measurement. Four packets is four servings.
- Notice what it does to your appetite. Some people find sweet-tasting drinks make the back half of a fast harder. If that is you, the sweetener is costing you more than its calories.
