Quick answer: At 2 weeks the scale is down 3–7 lbs and almost none of it is fat. At 1 month, 4–8 lbs on the scale of which 2–4 lbs is fat. At 3 months, 10–20 lbs of fat loss for people who stayed consistent — which in the controlled trials works out at 3–8% of starting body weight. The ranges are wide because the response really is that variable, not because nobody has measured it.

Three milestones account for almost every question people ask about intermittent fasting results: the two-week mark, where the first big number appears and nobody knows what it means; the one-month mark, where the first number stops appearing and people quit; and the three-month mark, where the change is finally large enough to see.

The three numbers, side by side

Two things about that table are worth more than the numbers in it.

The first is that the ratio inverts. At two weeks, roughly one pound in six is fat. At three months, roughly five pounds in six is. The most dramatic week of scale movement you will ever have is the one that changes your body composition least, and the weeks where the scale barely moves are the ones doing the work. Almost everyone who quits intermittent fasting quits during the second of those, having concluded from the first that it had stopped working.

The second is that these are midpoints of ranges that overlap. A well-run month for one person is a disappointing fortnight for another, and the difference is mostly not effort.

Why the ranges are so wide

Three factors do most of the explaining, and only one of them is under your control.

Starting weight. More stored fat means a higher resting burn and a larger deficit from the same protocol. Someone starting at 300 lbs may drop 20+ lbs in the first month; someone starting at 160 lbs might see 4–6 lbs over the same weeks while doing exactly the same thing. Both are normal.

Which protocol, and whether you kept it. A 16:8 schedule you hold for a year beats a 20:4 schedule you abandon in week three, every time. The controlled trials bear this out at the top end: the aggressive 4-hour and 6-hour windows tested by Cienfuegos and colleagues produced about 3.2% body-weight loss over 8 weeks — barely different from the 8-hour window Gabel's group tested over 12 weeks at 2.6%, and from the 10-hour window Wilkinson's group tested at around 3%. Narrowing the window past a point buys much less than people expect.

You. The PREDICT study put 1,002 UK twins and adults through identical meals and measured what happened afterwards. Between-person variation in the blood triglyceride response was 103% of the mean; glucose 68%; insulin 59%. Same food, same clock, wildly different physiology — and identical twins differed from each other too. There is no version of this page that can narrow your range to a single number, and any page that gives you one is guessing.

What you control is the food inside the window, the protein, and whether you are still doing it in twelve weeks. That is most of the outcome.

At 2 weeks: the number that fools everyone

The first fortnight produces the fastest scale movement of the entire process and the least fat loss.

The mechanism is glycogen. Your liver and muscles hold roughly 300–500 grams of it, and each gram is bound to 3–4 grams of water. Emptying those stores nightly releases something like 1–2 litres of water, which is where the 3–7 lb drop comes from. Lower insulin adds to it: the kidneys start excreting more sodium and water, which is also why headaches and cramps show up in week one and why they respond to salt rather than to food.

What else week 1 and 2 reliably bring, from our first week and week-by-week pages:

  • Hunger arriving on a schedule. Ghrelin fires at the times you trained it to. Each wave peaks and passes inside 20–30 minutes.
  • Day 3 being the worst day. Nearly everyone reports it. It is not a sign that fasting does not suit you.
  • Lighter sleep, as cortisol timing adjusts.
  • Less bloating, usually the first change people actually like.

If the scale goes up on day 2 or 3, that is normal fluctuation and not a verdict.

At 1 month: the plateau that is not a plateau

Month one on the scale looks like a cliff followed by a flat road, and the flat road is the point at which most people conclude it has stopped working.

It has not. Water loss is a one-time payment; it does not repeat in month two because there is no second reservoir to empty. What replaces it is fat loss at 0.5–1 lb a week, which on a bathroom scale is almost invisible under normal day-to-day fluctuation of two or three pounds. The whole month's genuine progress can hide inside the noise of a single weigh-in.

By the end of month one most people also report the thing that actually predicts whether they are still doing this in a year: the fast has stopped being difficult. Morning hunger has faded, the window closes without a fight, and the decision fatigue of three meals a day is gone.

At 3 months: where the trials and the mirror agree

Twelve weeks is the first point at which the answer is stable enough to trust, which is why nearly every controlled trial of time-restricted eating runs that long.

Across those trials the result clusters between 3% and 8% of starting body weight. Trepanowski's alternate-day fasting trial reached about 6% at six months — and, importantly, that was no better than plain daily calorie restriction over the same period. Fasting is a way of eating less that many people find easier to keep. It is not a separate metabolic mechanism that beats arithmetic.

In practical terms, three consistent months is 10–20 lbs of fat for most people, arriving at 1–2 lbs a week once the water is gone. This is also the stretch where the change becomes visible to other people before it becomes obvious to you, and where measurements start disagreeing with the scale in your favour: body composition improves faster than body weight if protein and training are in place.

The results that never show up on a scale

Weighing yourself measures one thing badly. The corpus tracks several others that arrive on their own schedule:

  • Insulin sensitivity improves within weeks, ahead of any meaningful fat loss.
  • Blood pressure and atherogenic lipids improved in Wilkinson's 10-hour window trial in patients with metabolic syndrome.
  • Steadier energy — fewer mid-afternoon crashes, once fat adaptation is underway.
  • Waist measurement often moves while the scale sits still, because visceral fat is not distributed like scale weight.
  • The relationship with hunger. By month three most people can tell hunger from habit, which is the change most likely to still be there in five years.

How to make the numbers land at the top of the range instead of the bottom

Two levers do most of the work, and both are about what happens inside the window rather than outside it.

Protein. The evidence-based range for protecting lean mass in a deficit is 1.6–2.2 g per kg of body weight per day. Below it, a share of everything you lose is muscle, and the scale rewards you for it while the mirror does not. Our page on muscle and fasting has the detail.

Pace. Elite athletes losing at 0.7% of body weight per week gained lean mass; those losing at 1.4% a week lost it, on identical protein and identical training. Faster is not better past a point, it is just more expensive.
