Does Bulletproof Coffee Break a Fast?
Quick answer: Yes. A standard butter coffee runs 220 to 450 calories — more than a bowl of cereal. Under a strict fast and a fat-loss fast it plainly breaks it. The "but fat doesn't raise insulin" defence is half true and does not rescue 450 calories. Butter coffee can still be a good tool; it is just a meal, and it should be counted as one.
Of everything people drink while fasting, this has the largest gap between how it is described and what it contains.
What is actually in the cup

The original recipe calls for one to two tablespoons of grass-fed butter plus one to two tablespoons of MCT oil. Run the arithmetic and a modest cup is about 220 calories; the full version is about 445 — drunk on an empty stomach by someone who believes they are fasting.
USDA has no analyzed entry for MCT oil in its SR Legacy or Foundation Foods tiers, so the coconut oil figure above is the closest published stand-in. MCT oil is essentially pure fat, so a tablespoon lands in the same neighbourhood.
Salted and unsalted butter are identical in USDA's data on calories, fat, protein and carbohydrate; the difference is sodium. "Grass-fed" is a farming claim, not a calorie one.
Three fasts, three answers

A strict clean fast — broken. 220 to 450 calories against a limit of zero.
A fat-loss fast — broken, and this is the one that costs you. The whole point of a fat-loss fasting window is that the calories you skip are calories you do not eat. Drinking 450 of them removes most of the benefit of skipping breakfast. The insulin argument below does not change the arithmetic.
An autophagy fast — assume broken. No human study has isolated dietary fat as a variable acting on autophagy, so nobody can quote you an effect size. What is not in doubt is that this is a substantial energy load, and energy availability is the signal fasting manipulates. If autophagy is your goal, this is not the drink for it. The fat-fasting argument, taken seriously
The case for butter coffee is not stupid, and it deserves a proper hearing before being taken apart. It goes: fasting works through low insulin; fat barely raises insulin; therefore a fat-only intake preserves the fasted hormonal state while removing the hunger. Fat is a "clean" fuel that keeps you in the fasted mode.
Two parts of that are testable.
Does fat really not raise insulin?
Less than carbohydrate, but not zero, and the "not at all" version is wrong.
A 2010 study in the Journal of Clinical Endocrinology and Metabolism gave subjects 25 g of fat — lard, olive oil or safflower oil — on its own, and reported that all three fats "modestly increased the insulin concentration when compared to ingestion of water alone."
The older finding is more interesting. In 1983, Collier and O'Dea gave eight subjects 50 g of butter alongside carbohydrate or protein. Blood glucose was lower with the fat present, as expected — but "the insulin response was not reduced, suggesting a potentiation of insulin secretion in the presence of fat," with greatly increased GIP. Fat did not quieten the insulin response; it amplified it relative to the glucose rise.
So fat is metabolically quieter than carbohydrate. It is not silent.
Does a high ketone reading mean a deeper fast?
This is the part almost everyone gets backwards, and it is the strongest reason to be skeptical of the whole premise.
MCT oil does raise blood ketones. In a crossover study in healthy adults, two 20 mL doses of tricaprylin (C8) raised mean plasma ketones by about 295 µmol/L across the day compared with control — real, and about a third of a millimole, not the multiple millimoles the marketing implies.
But a ketone reading tells you what is in your blood, not where it came from. The clearest demonstration comes from work on exogenous ketone drinks: they raised blood ketones sharply while lowering blood glucose, triglycerides and free fatty acids — with FFA falling from about 0.7 to 0.4 mmol/L. The authors concluded that exogenous ketones "inhibit adipose tissue lipolysis," and noted that the coexistence of low free fatty acids with high ketones is unique to exogenous ketosis.
Read that again, because it inverts the sales pitch. In a real fast, ketones are high because you are breaking down your own fat. After a ketogenic drink, ketones can be high while fat release is suppressed. A high reading is not proof of a deeper fast — it can be the signature of the opposite.
That experiment used ketone esters and salts rather than MCT oil, so it is not a direct test of butter coffee. The lesson transfers anyway: ketones in the blood can come from what you drank rather than from fat you mobilised, and a meter cannot tell the difference.
Coconut oil is not MCT oil
These get used interchangeably and they are not the same thing. USDA's fatty acid breakdown for coconut oil, per 100 g:
- Lauric acid (C12): 41.8 g
- Caprylic acid (C8): 6.8 g
- Capric acid (C10): 5.39 g
Commercial MCT oil concentrates C8 and C10 — the two fractions that make up only about 12 g per 100 g of coconut oil. Coconut oil is dominated by lauric acid, which behaves more like a long-chain fat. This is why the crossover study above found tricaprylin alone raised ketones more than coconut oil did.
If someone tells you a spoon of coconut oil in your coffee does what MCT oil does, the fatty acid profile says otherwise.
What the one real trial found
Butter coffee has barely been tested, but it is not untested. A 2024 randomized trial ran 12 weeks of plain coffee against coffee with butter and MCT oil, with 41 people completing it. Its primary outcome was ApoB, a marker of cardiovascular risk, and it found no difference between groups at baseline or at 12 weeks — concluding that adding MCT oil and butter to coffee "may be safe."
That is a useful and limited result. It says butter coffee did not wreck these people's lipid markers over three months. It says nothing about fasting: no trial has compared butter coffee against an actual fast on any fasting-relevant outcome. Anyone claiming it "keeps you in a fasted state" is filling that gap with assertion.
Who butter coffee actually suits
Dropping the fasting claim leaves something genuinely useful:
- You are on a ketogenic diet and this is breakfast. Then it is a fine breakfast. Count the calories and carry on.
- You would otherwise eat a 700-calorie pastry at 9am. Swapping that for a 250-calorie coffee is a real improvement. It is still eating.

Butter coffee is a low-carb meal replacement, not a fasting aid. Used that way it is fine. Used as a way of "fasting" until 2pm, it is a 450-calorie accounting error.

- Want the fast intact? Black coffee — 2 calories, and about 95 mg of caffeine per 8-ounce cup.
- Want the appetite help without the calories? Salt in water, or coffee with cinnamon.
